However, please submit the day that works best for you, as well as if you would like to come in in the morning or afternoon (AM/PM). Once one of our staff has received your information we will contact you to confirm an exact time and date for your appointment.

If you are a new client to Stanford Hearing Aids, feel free to print and fill out our "Patient Information Form" and our "Hearing Health Assessment" prior to your appointment time. This will save you time during the appointment.
